I'm not sure if it's a bug or not, but I'm reporting it as a bug and suggesting an improvement. I'm playing in free mode with a custom configuration, and the difficulty level is set to Madman. I've been playing for a few days, and I don't see any difference between the two difficulty settings. When riding a four-wheeler on the mountains, if you fall down a slope or a high hill, you should die because you fall downwards at a higher speed, but you don't die, and there's only damage to the helmet. Running or jumping on hills should also result in death, but it only causes damage to the helmet. The same thing happens when going downhill on mountains; only helmet damage occurs. I only die when the bike flips upside down and the character crashes into a rock, mountain wall, or the ground when falling upside down. It rarely happens, and almost never do I die. Helmet damage is suitable for minor falls, but not for significant falls where death should occur. There should also be damage from rocks when collecting them, with minor damage from small rocks and death if a large rock falls on you. Games like Icarus, Subsistence, Subnautica, and others have different levels of damage and realistic details. The rest of the equipment and quantity difficulties, I don't see much difference. For example, the first aid kit contains the same amount in Madman and Colony. My advice is to review it thoroughly. I'm reporting this as a bug and suggesting an improvement.
